Sample Identity Theft Letter

If you are a victim of identity theft or identity fraud, you need to send a letter to each of the three credit reporting companies asking them to block or correct fraudulent information in your credit file.

Please note that you will need to write a separate letter to each reporting company. Telephone each company at the telephone number indicated, and ask the customer service representative for the email or mailing address where you should send dispute or blocking requests.

Here is a sample dispute letter. The bracketed information can be edited to reflect your personal circumstances.

Dear Ladies/Gentlemen:

I am a victim of identity theft and am writing to dispute certain information in my file resulting from the crime. Please see the attached copy of my credit report on which I have circled the items I dispute. I did not make or authorize any of the disputed items.

Please remove or correct this information at the earliest possible time. I dispute the [name of source, like “Company” or “Court”] [name of item, like “account” or “judgment”] because [I did not make or authorize this transaction] [I did not open or authorize this account] [other explanation why the item is not accurate].

As required by section 611 of the Fair Credit Reporting Act, a copy of which is attached, I hereby request that the item[s] be removed [or other requested change] to correct the information in my file. {you can print out and attach a copy of Section 666 of the Fair Credit Reporting Act]
